Model Consent Form

Please complete the Bliss AVR model consent form before scheduling a shoot.

To ensure legal compliance, Bliss AVR must keep a copy of a valid photo ID displaying the model’s date of birth.

Please submit the form, along with a photo of your ID (ID card, driving license or passport) and your face next to ID (like photo below), and we’ll contact you to arrange the shoot. Your ID must show your photo, name and date of birth.

All fields are mandatory.

These details will be held securely and will not be shared with anyone except the relevant authority on request.
